Myosin Heavy Chain Myopathy (MYHM) is associated with a variant in the MYH1 gene, which encodes myosin heavy chain 2X, a protein essential to fast-twitch muscle fiber function. Immune stimulation, including certain vaccinations or recent infections, may act as a trigger for IMM in genetically susceptible horses. This does not mean vaccines are inherently unsafe; rather, knowing a horseโ€™s genetic status can help veterinarians make informed management decisions.

In Quarter Horses and related breeds, this genetic variant is associated with two distinct muscle disorders:

๐ˆ๐ฆ๐ฆ๐ฎ๐ง๐ž-๐ฆ๐ž๐๐ข๐š๐ญ๐ž๐ ๐ฆ๐ฒ๐จ๐ฌ๐ข๐ญ๐ข๐ฌ (๐ˆ๐Œ๐Œ): an autoimmune condition that can cause stiffness, weakness, and severe, rapid muscle atrophy, particularly affecting the gluteal and epaxial (back) muscles. Lymphocytic inflammation may be present within muscle tissue and surrounding blood vessels.

๐๐จ๐ง-๐ž๐ฑ๐ž๐ซ๐ญ๐ข๐จ๐ง๐š๐ฅ ๐ซ๐ก๐š๐›๐๐จ๐ฆ๐ฒ๐จ๐ฅ๐ฒ๐ฌ๐ข๐ฌ: a form of muscle-cell breakdown that can cause pain, stiffness, swelling, and elevated muscle enzymes without exercise being the trigger. Unlike IMM, significant muscle atrophy and lymphocytic inflammation are not typically present.

Although these conditions present differently, risk for both is associated with the same MYH1 genetic variant. Not every horse carrying the variant will develop clinical disease, but genetic status can provide valuable information for health management and breeding decisions.
