Explanation - This is a Vet BLUE® classic having "wet lung" symmetrically in the upper non-gravity dependent Vet BLUE® Caudodorsal and Perihilar lung regions.

Answers – This is a Vet BLUE® "Wedge Sign." The "Wedge Sign" is 1 of the 4 Vet BLUE® Signs of consolidation that also include the "Shred Sign", "Tissue Sign", and "Nodule Sign."

A "Wedge Sign" is hallmarked by the "wedge-shaped" periphery finding off the "Lung Line" that is an amalgam of the "Shred Sign" and "Tissue Sign" and when present where pneumonia is unexpected, i.e., Vet BLUE® Caudodorsal and Perihilar views, then pulmonary thromboembolism should be considered in the differential.

This case was one of the author's cases in 2018 and published as Lisciandro G, Puchot M, Gambino J, and Lisciandro S, in JVECC 2021.

This is a Vet BLUE® "Nodule Sign." The "Nodule Sign" is 1 of the 4 Vet BLUE® Signs of consolidation that also include the "Shred Sign", "Tissue Sign", and "Wedge Sign."

A "Nodule Sign" is hallmarked by a dark (hypoechoic to anechoic) circular or oval structure(s) in contact with the "Lung Line" that moves with phases of respiration and has a "Pseudo B-line" extending from its far border through the far field.

This finding supports these differentials not in any order - 1) granulomas due to fungal disease, verminous disease (heartworm in dogs, lungworms in cats), and less commonly bacterial rarely viral, 2) neoplastic, and 3) abscess from inhaled or migrating foreign body, or penetrating wounds.

The term "Pseudo B-line" has been published by the author and used when non-fluid substrates result in this vertical artifact, i.e., soft tissue nodules, and semi-solids as kibble in the stomach, "Pseudo-gastric B-lines."

"True B-lines" include the author's mantra of "water, blood or pus" in other words "water" as cardiogenic and non-cardiogenic pulmonary edema, "blood" as lung hemorrhage including lung contusions in trauma, and "pus" as pneumonias.

Answers – A "Tissue Sign" is hallmarked by the absence of bright white (hyperechoic) punctate and linear areas within the lung parenchyma that represent air bronchograms making lung appear like liver (called "hepatization of lung" in older radiology textbooks), thus the term "Tissue Sign."

This finding supports these differentials not in any order - lung lobe torsion, severe pneumonia, neoplasia, and severe atelectasis.

Some Important Points: The depth setting is cutting off the pathology in the far-field. Have enough depth by clearly seeing the far-field and periphery of the suspect pathology. Using our rule of thumb for depth is "L-U'N'G" has 4 letters so use 4-8cm as a range of depth for smaller to larger patients. In larger dogs even 9-11cm is appropriate. In this manner artifacts and pathology may be seen through the far-field. In too close, too shallow depth, risks misinterpretations.

Note the date of February 8, 2014, well before anyone in veterinary medicine was using proactive lung ultrasound as part of the physical exam.
