What is TPE?
This is a long post but it’s what saved Romolo’s life🤎
TPE is "total plasma exchange", “therapeutic plasma exchange”, “plasmapheresis”. It’s all the same and it saved Romolo’s life!! It’s kinda like when a person has dialysis. The blood is made up of red blood cells and plasma. Antibodies are in the plasma, so the goal is to remove the antibodies in bloodstream.
TPE only been published in a journal once (as per Romolo’s neurologist) for MG; it is typically used for other autoimmune conditions like immune-mediated anemia. It is not something vets rush to doing, unless a dog comes in severely affected like Romolo was.
We were given 3 choices the night we arrived in the ED. Romolo unfortunately aspirated and was dx with aspiration pneumonia 🥲
He was having difficulty breathing and he was extremely weak.
1. Continue doing what we were doing for the prior 3 months. Which was not working
2. Say goodbye, and euthanize Romolo
3. Attempt TPE. The neurologist could not guarantee that it would work or how long it would last.
I had fought so hard the past 3 months and my father died a few days prior. I was a complete mess. I was NOT ready to give up!!!!
Romolo was too weak to travel to NC for TPE. Dr. Kopec explained she would do it manually. They call their version "ghetto TPE" because we do not have a plasmapharesis machine (think like a dialysis machine) that filters the plasma constantly.
My husband was very skeptical when he heard “ghetto TPE”!!! He couldn’t let me lose my dad and my dog within days of each other. We decided to try one last thing to save Romolo.
The neurology calculated Romolo's total blood volume based on his weight, they removed 1/4 of his blood volume (about 500 mLs) for each treatment. We then spun the blood in a centrifuge to separate the red blood cells from the plasma, they drained off the plasma and threw it away (since that is where the antibodies were), we gave him back his red blood cells, and then they gave him a plasma transfusion.
They did this 4 times over about 2 days to completely replace his plasma, but he responded after the first treatment. Romolo arrived in ED hardly able to walk and with aspiration pneumonia/constantly regurgitating and after the first treatment was no longer regurgitating and was much stronger.💕click on his picture for details.
I am so grateful for this technology, Dr. Kopec, UGA, and that we agreed to doing it!!
