English below (intubation technique in Chinchilla)
การสอดท่อช่วยหายใจคือจุดเริ่มต้นของความปลอดภัยในทุกๆ หัตถการ หรือการผ่าตัด
ชินชิล่า เป็นสัตว์ที่สอดท่อช่วยหายใจได้ยากที่สุดชนิดหนึ่ง เพราะด้วยกายวิภาค หรือ Anatomy ของเค้า ช่องปากที่เล็ก ลิ้นที่ใหญ่(แต่ไม่เท่าหนูแกสบี้ (cavy)) และ พื้นที่มีจำกัด ที่ AnimalSpace เราใช้ 3 เทคนิค ในการสอดท่อ ค่อยๆไล่ดูทีละคลิปก็ได้ครับ ว่าแต่ละวิธีทำกันยังไง เริ่มจาก
1. Blind intubation คือการสอดด้วยวิธีนี้ เน้นที่เข้าใจสรีระและการจัดตำแหน่ง ทำจนคุ้นชิน และ วิธีนี้จะเร็วที่สุด
2.Scope assist การใช้ Endoscope ช่วยข้อดีเหมาะสำหรับมือใหม่ แต่ รวย 😂😂 เพราะเครื่องที่ใช้ ซื้อรถได้หนึ่งคัน 😅 ข้อดีคือมองเห็นกล่องเสียงและหลอดลม ทำให้สอดท่อได้ง่ายขึ้น (เหรอ)
3. Direct Visualize หรือใช้อุปกรณ์ช่วยส่อง คล้ายๆกับแบบที่ 2 แต่บางครั้งก็ประสบความสำเร็จ บางครั้งก็ไม่
ถามว่าแบบใดดีที่สุดก็คงตอบว่าขึ้นกับสถานการณ์ ในการเลือกใช้อุปกรณ์และเทคนิค ในการสอดท่อช่วยหายใจ ที่แตกต่างกันไปในแต่ละเคส แต่ !!!!! ไม่ว่ายังไง ก็ต้องสอดท่อช่วยหายใจ เพื่อป้องกันการหยุดหายใจระหว่างผ่าตัด ซึ่งมีโอกาสเกิดขึ้นได้ แต่หากเราสอดท่อช่วยหายใจอยู่ เราสามารถเปิดเครื่องช่วยหายใจได้
🫁 Chinchilla Intubation: 3 Techniques I Use
Intubating a chinchilla can be challenging. Their oral cavity is small, the tongue takes up a lot of space, and visualization of the glottis can be quite limited.
Here are 3 techniques I commonly use:
1️⃣ Blind Technique
Direct visualization of the glottis is not required. Proper positioning, a good understanding of the anatomy, and tactile feedback are the keys. Once mastered, this technique can be surprisingly quick.
2️⃣ Scope-Assisted Technique
A small scope helps identify and guide us toward the glottis. This provides better visualization, especially when the airway is difficult to access.
3️⃣ Direct Visualization with a Laryngoscope
A laryngoscope is used to visualize the glottis directly, allowing the endotracheal tube to be advanced into the trachea under direct vision.
There is no single technique that works best for every patient.
The key is to know different techniques and choose the one that best suits the patient in front of you. 🤍
