Feline Idiopathic Cystitis (FIC)
FIC is a common cause of feline lower urinary tract disease (FLUTD), characterized by sterile inflammation of the urinary bladder with no identifiable cause such as uroliths, infection, or neoplasia.
Etiology / Pathogenesis
Exact cause unknown (idiopathic)
Associated with stress, indoor lifestyle, obesity
Neurogenic inflammation, defective glycosaminoglycan (GAG) layer
Increased bladder permeability and abnormal stress response (↑ catecholamines)
Clinical Signs
Dysuria, stranguria
Pollakiuria
Hematuria
Periuria (urinating outside litter box)
Vocalization; may cause urethral obstruction in male cats
Diagnosis
Diagnosis of exclusion
Urinalysis: hematuria ± proteinuria, no bacteria
Imaging to rule out stones or masses
Treatment / Management
Stress reduction (environmental enrichment, litter hygiene)
Increase water intake (wet diet, fountains)
Analgesia (buprenorphine)
Antispasmodics if needed
Avoid unnecessary antibiotics
Prognosis
Generally good
Recurrent episodes common; managed rather than cured
👉 Key point: FIC is stress-related, non-infectious cystitis and antibiotics are NOT indicated unless infection is proven.
