Pyometra is a serious uterine infection that can threaten a dogโs life if not diagnosed and treated promptly.
๐ Patient Profile:
A 9-year-old female Shih Tzu presented to our clinic. The owner was concerned that the dogโs butt had been bleeding all night, but upon careful assessment, the bleeding was actually coming from the va**naโa key sign of pyometra.
๐ What is Pyometra?
Pyometra is the accumulation of pus in the uterus, usually affecting intact female dogs. It often occurs after heat cycles due to hormonal changes and bacterial infection.
๐น Types of Pyometra:
Open Pyometra โ The cervix is open, allowing pus to drain from the va**na. Owners may notice va**nal discharge, as in this case.
Closed Pyometra โ The cervix is closed, trapping pus inside the uterus. This can be more dangerous, as it may progress without obvious discharge.
๐ฌ Why Ultrasound is Crucial:
In a normal uterus, the organ is tiny and usually not visible on ultrasound.
In pyometra, ultrasound shows:
Enlarged, fluid-filled uterus
Thickened uterine walls
Echogenic (pus-like) contents
On ultrasound, the organ on the left side is the urinary bladder, which appears black (anechoic) due to fluid content, helping differentiate it from the uterus.
Ultrasound allows early detection, even in closed pyometra where discharge isnโt visible.
๐งช Laboratory Findings:
CBC:
โ WBC, โ LYM #, โ MID #, โ GRA #, โ PCT (signs of infection/inflammation)
โ RBC, โ HGB, โ MCHC, โ MCH, โ HCT, โ PLT, โ PCT (anemia and low platelets)
Blood Chemistry:
Within normal limits
C*C Test:
Normal
โ ๏ธ Clinical Signs:
Vaginal discharge (may be purulent or bloody)
Lethargy and loss of appetite
Increased thirst and urination
Vomiting or abdominal pain
In this case, owner-perceived a**l bleeding actually originated from the va**na
๐ก Why It Wasnโt Just โDog Menstruationโ
Dogs do not have menstrual periods like humans.
Bleeding from pyometra is often continuous, purulent, or foul-smelling, unlike the brief spotting seen in heat.
At 9 years old, this dog was likely past normal estrus cycles.
Pyometra may also cause lethargy, vomiting, or increased thirst, which are not seen in a normal heat cycle.
๐ Management:
Surgical removal of the uterus and ovaries (OHE/OVH) is the treatment of choice
Supportive care: IV fluids, antibiotics, and monitoring for complications
๐ Preventive Advice from Our Clinic:
If you do not plan for your dog to have puppies, we strongly recommend OVH (spay surgery). This not only prevents unwanted litters but also dramatically reduces the risk of life-threatening conditions like pyometra.

Pyometra can develop silently, especially the closed type, but ultrasound makes it easy to detect before it becomes life-threatening. Owners should monitor for any va**nal discharge, even if it seems like a**l bleeding.
