AVCI recently gathered our veterinarians from across our branches for another veterinary team meeting—a time to discuss clinical updates, review practices and protocols, share experiences from different branches, and keep everyone aligned in the way we care for our patients.

An Unexpected Finding in Cebu: Suspected Canine Leishmaniasis

A dog presented to AVCI with months of persistent hair loss that initially involved itching, although the itching had become much less frequent while the hair loss continued, along with mild corneal opacity.

Routine testing showed dermatophytes, but an unexpected finding stood out: the Leishmania antibody test was positive twice.

Because canine leishmaniasis is not considered endemic in the Philippines, the result is unusual and requires further confirmation. Lymph node cytology also showed suspicious structures, but the case remains suspected—not confirmed.

Cases like this are a reminder that infectious diseases may appear outside the places where we usually expect them, especially as animals, people, and vectors move between regions.

WHEN ONE TEST ISN’T ENOUGH: A BABESIOSIS CASE AT AVCI

This patient was brought to AVCI because of eye discharge and poor appetite.

As part of the initial diagnostic work-up, tick-borne disease rapid screening and a CBC with peripheral blood smear (PBS) examination were performed together. The rapid screening test returned negative, while microscopic examination of the blood smear revealed Babesia organisms inside the red blood cells.

Why can this happen?

The tick-borne disease rapid test used in this case is an antibody-based test. This means it does not directly look for the Babesia organism itself. Instead, it detects antibodies produced by the patient’s immune system in response to infection.

During the earlier stages of infection, Babesia organisms may already be circulating in the blood, while the body may not yet have produced enough detectable antibodies for the rapid test to register positive. This is sometimes referred to as the serologic window period. Test performance may also depend on the particular Babesia species or organisms the kit is designed to detect.

A peripheral blood smear provides a different type of information. It allows the blood cells themselves to be examined directly under the microscope and, when organisms are present in sufficient numbers, Babesia may be seen within the red blood cells.

At AVCI, we believe the word “complete” in Complete Blood Count should go beyond simply obtaining numbers from a machine. As part of our CBC work-up, the blood smear is also examined microscopically, allowing our veterinary team to assess the blood cells more closely and look for findings that may provide additional clinical information.

Laboratory findings are then interpreted together with the patient’s clinical signs, physical examination, and other appropriate diagnostics rather than relying on any single test alone.

We also appreciate our patient’s furparent for bringing the pet in early and allowing the recommended diagnostic work-up to be performed. This gave our veterinary team the opportunity to identify the problem promptly, begin appropriate treatment, and closely monitor the response.

Fortunately, the patient responded well and was already showing noticeable clinical improvement by Day 3 of hospitalization. When the patient returned to AVCI for follow-up, the patient was clinically recovered.

Sometimes, the numbers are only part of the CBC. Looking at the blood itself can tell us more.

Sharpening Our Skills: Ultrasound-Guided Procedures at AVCI

Behind the scenes, the AVCI team is always working to improve the medical care we give your pets! 🐾

Our veterinarians and vet techs recently completed a hands-on training session for ultrasound-guided sample collection. Using specialized synthetic models, the team worked on tracking targets and guiding needles with pinpoint accuracy.

This training prepares our staff for vital clinical procedures like ultrasound-guided cystocentesis, abdominocentesis, and biopsies. Practicing on simulated models helps us build solid hand–eye coordination and clinical confidence in a controlled environment before working with live patients.

Not All Mange Is the Same — But This One Can Make People Itch Too

When people hear “mange,” they often think it is just one skin disease. In dogs, however, mange can be caused by different mites, and they do not all behave the same way.

Two common types are demodectic mange and sarcoptic mange. Demodectic mange is generally not contagious to people, while sarcoptic mange is highly contagious between dogs and can also cause very itchy red skin lesions in humans.

The pet was only 3 months old when she developed persistent scratching, hair loss, crusting, redness, and irritated skin. Proper skin testing helped identify the cause as sarcoptic mange.

So when a dog is constantly scratching, developing bald patches, crusts, redness, or wounds, it is better not to assume that it is simply an allergy. And if people in the household are starting to itch too, that is an even stronger reason to have the pet examined promptly.

After treatment and follow-up, the puppy showed marked improvement, with much less scratching, clear hair regrowth, and no open wounds or active pyoderma by her later examination.

Watch the video closely — that tiny moving mite is Sarcoptes. Small enough to miss with the naked eye, but capable of making both pets and people very uncomfortable.

Not every mange is the same. Finding out which mite is involved matters because the treatment, contagiousness, and risk to the household can be very different.

When Bad Breath Is a Warning Sign, Not Just a Nuisance

This pet came in because of a problem many furparents may be tempted to dismiss at first: very bad breath.

But in her case, it was already a sign of severe dental disease. At 13 years old, she had heavy plaque and tartar buildup, receding gums, loose and badly supported teeth, and chronic infection in her mouth.

Before the dental procedure, she underwent a physical exam, blood test, and ECG to make sure she was stable enough for anesthesia. She was then placed under gas anesthesia for dental cleaning, polishing, and removal of teeth that could no longer be saved.

One important finding came during the extraction of one of her upper canine teeth. The dental disease had become so advanced that it had already created an abnormal opening between the mouth and the nasal cavity. This had to be cleaned and surgically closed during the procedure.

Her case is a good reminder that dental disease is not only about dirty teeth or unpleasant breath. When it is allowed to progress, it can damage the gums and the structures holding the teeth in place, cause chronic infection, and sometimes create much bigger problems inside the mouth.

So when your pet’s breath becomes unusually foul, the gums are receding, chewing seems uncomfortable, or the teeth are covered in thick tartar, it may be time for a dental check—not just another dental chew.

AVCI branches remain open during power interruptions, with standby generator backup in all branches and solar power already supporting three branches.

These systems help us keep clinic operations running as normally as possible during brownouts. Consultations, vaccinations, deworming, laboratory testing, diagnostic imaging, treatments, procedures, surgeries, and other regular veterinary services continue across AVCI.

For admitted patients, monitoring, medications, treatments, and nursing care also continue throughout these periods.

So even during a brownout, you can still bring your pet in for veterinary care as you normally would.

When Vaginal Discharge Keeps Coming Back, Look Deeper

This 16-year-old unspayed dog was brought in for recurring vaginal discharge together with persistent signs of heat. Because she was already a senior and also had chronic kidney disease, her condition required careful assessment and treatment planning.

One important finding was that her X-ray did not show the very enlarged, fluid-filled uterus that many people associate with pyometra.

This was because she had an open pyometra.
In open pyometra, the cervix remains open, allowing infected material from the uterus to drain out as vaginal discharge. Because the fluid is able to escape, the uterus may not appear dramatically enlarged on X-ray.

Her history, laboratory tests, vaginal cytology, and imaging all helped confirm the reproductive problem. Surgery was performed, and during the ovariohysterectomy, multiple uterine and ovarian polyps were also found and removed.

For fur parents, this case is an important reminder:
👉 Recurring vaginal discharge in an unspayed dog should never be ignored, especially in senior pets.
👉 Pyometra does not always cause a visibly enlarged uterus on X-ray.
👉 Discharge that seems to come and go may still be a sign of an ongoing uterine problem.
👉 Senior pets and those with other medical conditions may need a more carefully planned approach to surgery and recovery.

Following surgery, her reproductive condition was resolved, while her chronic kidney disease continues to be managed with maintenance care.

Sometimes the warning sign is not how dramatic the discharge looks, but the fact that it keeps returning. 🐾

If your unspayed female dog develops unusual or recurring vaginal discharge, have her examined by a veterinarian as soon as possible.
