EMERGENCY CESAREAN SECTION DUE TO FELINE DYSTOCIA
An adult pregnant mixed-breed cat was brought to our clinic for queening assistance. According to the owner, the patient had been in labor for approximately two days but was having difficulty delivering the kittens. This was also her first pregnancy.
Upon presentation, the patient had foul-smelling, purulent vaginal discharge, raising concern for severe reproductive tract infection and compromised fetal viability. Given the prolonged labor, difficulty delivering the kittens, and concerning vaginal discharge, the case progressed to an emergency Cesarean section (CS).
During surgery, two deceased kittens were retrieved. The kittens were already decomposed, without intact fetal sacs, consistent with prolonged intrauterine retention and fetal death.
This case highlights an important emergency in feline reproduction: DYSTOCIA—difficulty or inability to successfully deliver the kittens through the normal birth canal.
WHY DOES DYSTOCIA HAPPEN IN CATS?
Feline dystocia may occur due to problems involving the mother, the kittens, or both.
Maternal causes may include:
• Weak or ineffective uterine contractions (uterine inertia)
• Exhaustion following prolonged labor
• Abnormalities or narrowing of the birth canal
• Pelvic abnormalities or previous pelvic trauma
• Uterine abnormalities or disease
Fetal causes may include:
• Oversized kittens
• Abnormal fetal position or presentation
• Fetal malformation
• A kitten becoming lodged in the birth canal
• Multiple fetuses positioned in a way that obstructs delivery
Other factors, including large litter size, very small litters, maternal stress, nutritional problems, and certain reproductive abnormalities, may also contribute to difficult labor.
Prolonged or abnormal labor should never be ignored! When a queen has been actively straining without delivering a kitten, becomes weak or exhausted, develops abnormal or foul-smelling vaginal discharge, or shows signs of pain or distress, immediate veterinary evaluation is necessary. Early assessment can help determine whether continued medical management is appropriate or whether an emergency Cesarean section is necessary to protect the queen and, when possible, save the kittens.
