When was the last time you went “fishing” for a heartworm?

Today was supposed to be my day off. But yesterday, a dog was referred to us for possible intervention. The situation was serious as heartworms had made their way into the right atrium, while severely increased pulmonary pressure had already led to right-sided heart failure.

The dog had been stabilised at an emergency centre, but there was still a problem that needed to be dealt with.

I immediately reached out to my teammates, and thank God, they were available and willing to come in to help.

This morning, we gathered for a mission: go fishing to retrieve the heartworm from the right atrium.

One hour later… mission accomplished.

The procedure went smoothly, and the heartworm was successfully removed.

Sometimes, you plan for a quiet day off, and suddenly you find yourself standing in a procedure room, surrounded by your team, dealing with a life-threatening case.

But these are the moments that remind me why teamwork matters so much. Huge thanks to my amazing teammates for answering the call when I needed them. 🙏🏻

This is my first time giving a non-cardiology-related talk to my fellow colleagues at the Malaysian Veterinary Medical Association (MAVMA).

It’s a sharing session titled “The Defamation Without a Face.”

Honestly, I found this topic much harder to deliver than my usual cardiology topics, perhaps because it is less about clinical knowledge and more about navigating something that can affect all of us as professionals.

Veterinarians today are living in an era where defamation may sometimes feel inevitable. I hope that, as professionals, we continue to uphold the highest standards of integrity, professionalism, and respect, both in our practice and in the way we communicate with one another.

Hopefully, this sharing session will encourage us to think, reflect, and approach these situations with greater awareness and wisdom.

Thank you MAVMA for inviting me.

Malaysia — Singapore

A return visit to Singapore for a productive training session at Canopy Veterinary Centre.

It was rewarding to work with the veterinary team and support their development of basic echocardiography skills. Seeing vets build confidence in cardiac imaging and expand their diagnostic capabilities is an important step towards improving patient care.

The trip was made even more memorable by celebrating my birthday in Singapore. I was asked why I would choose to spend my birthday working. For me, sharing knowledge and helping colleagues in the region develop their skills is one of the most meaningful ways I could spend my birthday.

And, of course, a good dinner with colleagues was the perfect way to round off the day.

It was a fulfilling trip professionally, and a memorable way to mark another year.

I was once asked by a pet owner,

“What machine do you use for echocardiography?”

It made me wonder:
Will pet owners one day ask what C-arm machine is being used to X-ray their pets?

Perhaps they should. Because, exposure matters.

We’re not talking about a few seconds of exposure like radiography. We’re talking about minutes, and sometimes even hours, of continuous X-ray aka fluoroscopy.

Fluoroscopy is not simply about producing an image. The equipment used, its intended application, exposure settings, and duration all matter when it comes to radiation safety.

If fluoroscopy is performed using a machine that was not designed or intended for prolonged fluoroscopic exposure, what does that mean for the pet being directly exposed to the X-ray beam?

And what does it mean for the operators who are standing nearby, procedure after procedure?

Did you know that interventional cardiologists have a high risk of developing brain tumour?

We ask questions about image quality.
Perhaps we should also be asking questions about radiation safety. Because when it comes to X-rays, what you cannot see may matter just as much as what you can.

When I was asked by Malaysian Association of Veterinary Anesthesia and Critical Care (MAVACC) to give a cardiology talk, I chose a fundamental topic: understanding congestive heart failure.

I knew the webinar would have an audience with different levels of experience, including veterinarians, veterinary nurses, and students. So rather than focusing primarily on how we diagnose or how we treat congestive heart failure, I wanted to start with something more fundamental, something I rarely talk about in my past lectures.

Many people might have these questions: How do we diagnose it? How do we treat it?

I think before we can answer them, we need to understand what is actually happening in the first place - the underlying mechanism of congestive heart failure.

A good understanding of the pathophysiology gives us the foundation to understand why patients develop the clinical signs we see, why certain diagnostic findings occur, and ultimately, why we choose particular treatments.

With only 30 minutes to talk, I didn’t have enough time to go deeply into diagnosis and management. So I focused on one fundamental concept: left ventricular filling pressure.

Understanding what happens to left ventricular filling pressure—and why it increases—helps us connect the dots between cardiac dysfunction, congestion, clinical signs, diagnostic findings, and the rationale behind treatment.

I hope the attendees took away something useful from the short session, even within the limited time.

Thank you, MAVACC, for inviting me and for the opportunity to share.

Thank you, Sin Chew Daily, a Malaysian Chinese newspaper, for featuring me and highlighting the work of veterinary cardiologists in Malaysia.

Veterinary specialist services in Malaysia are still in their infancy, but awareness and access are growing. The establishment and development of organisations such as the Malaysian College of Veterinary Specialists are important steps towards recognising, developing, and promoting veterinary specialist services in Malaysia.

Pet owners should know that specialist care is available when their pets require more advanced diagnostics, treatment, and management.

I’m grateful for the opportunity to share more about veterinary cardiology and, hopefully, help raise awareness of the specialist services available to improve the health and quality of life of our beloved pets.

【From Kuala Lumpur to Jakarta: A Journey of Hearts, Passion, and Partnership】

I am honoured to have been invited by PET+VET Animal Clinic in Jakarta, Indonesia, to conduct a 3-day, 2-night training session for their veterinary team, focusing on several requested cardiology topics.

PET+VET is a veterinary clinic group in Indonesia with a strong focus on providing comprehensive veterinary care and developing clinical expertise within its team. It was a pleasure to spend time with their vets and have the opportunity to share knowledge, exchange experiences and learn from one another.

The session was not just about echocardiography. We focused on how to approach a cardiac case as a whole — starting with a thorough history and clinical assessment, followed by the appropriate use and interpretation of electrocardiography, echocardiography and Doppler evaluation. We also worked through several clinical cases together, discussing the thought process behind each case and how to integrate the different findings to arrive at a more meaningful clinical assessment.

The vets also had the opportunity for hands-on practical experience, allowing them to put some of the concepts discussed into practice.

Beyond the training itself, I’m especially happy that this provided an opportunity to foster collaboration and knowledge exchange between veterinarians from Malaysia and Indonesia. I believe that sharing experiences, cases and different clinical perspectives can help strengthen veterinary practice and cardiology in our region.

I sincerely hope the team found the training useful and took away something that they can apply in their daily practice and, ultimately, to benefit their patients.

A big thank you to the PET+VET team for the warm welcome, engaging discussions and wonderful hospitality. I’m truly honoured to have been invited and grateful for the opportunity to share, learn and connect with such an enthusiastic group of vets.

PET+VET

This 3-year-old Pomeranian presented with a persistently low resting heart rate. He also has an undescended testicle requiring surgery, prostatic enlargement, and an elevated ALKP liver enzyme, so his unexpectedly low heart rate was an important concern before anesthesia.

So, we performed atropine response test.

What is atropine response test❓

Why did we perform the test while he was sitting⁉️

Because he really did not want to be held in lateral recumbency. 🥲

Placing 4–5 ECG clips on such a small, 3kg restless patient was also challenging. Therefore, we used , which requires only 2 clips.

💡For a complete diagnostic ECG assessment, this setup is not ideal. However, in this particular patient, it provided a practical way to monitor his heart rate and assess how it responded to atropine.

Before atropine, his heart rate was 69 bpm.

Following atropine administration, his heart rate increased dramatically to approximately 250 bpm.

This strong chronotropic response supports the possibility that his resting bradycardia is primarily vagally mediated (extracardiac) rather than being caused solely by an inability of the heart’s pacemaker to increase its rate.

That is encouraging news for his upcoming surgery. It suggests that atropine may be effective in increasing his heart rate if clinically significant bradycardia occurs during anesthesia.

🚨However, this does not mean that anesthesia is completely risk-free. His underlying conditions and baseline bradycardia still warrant careful anesthetic planning, continuous monitoring, and readiness to manage cardiovascular changes during surgery.

Sometimes, good clinical decision-making means adapting our diagnostic approach to the patient in front of us—balancing ideal methodology with what is practical, safe, and clinically meaningful.
