We have re-examined all the medical documents you provided and thoroughly reviewed your policy’s terms and conditions in light of your request and we noted that the below mentioned documents are not made available to us which are mandatory for further processing of the claim. As due to non submission of claim documents “claim status is rejected”. To reconsider your claim kindly share below mentioned document as still pending-
1. Kindly provide Copy of Indoor case papers including the initial assessment sheet, doctors notes, nursing notes, daily Vital/ TPR/Spo2 chart duly certified by hospital authorities.
2. kindly provide UGIE(Upper Gastrointestinal Endoscopy) report.
We would request you to please submit the documents as mentioned above within 2 days from receipt of this mail, so that we can proceed further and process the claim. On receipt of the same we shall be able to evaluate and submit our final decision on admissibility of claim as per policy terms and conditions.
