Eosinophilic keratitis can be tricky to treat at the best of times, especially when the cornea is also ulcerated and topical steroid treatment is contraindicated.

This patient was treated with 0.2 mg/kg triamcinolone (Kenalog) via subcutaneous injection along with topical cidofovir q 12 hours and topical chloramphenicol ointment q 8 hours initially. The second image was taken 14 days later and there was a marked difference in corneal clarity and the ulceration had resolved. Topical tacrolimus was initiated q 12 hours and the chloramphenicol ointment was stopped. It is hoped that a further improvement will be noted with regards to the remaining corneal vascularisation and that eventually over time, clinical remission can be achieved as documented in the literature 🐈‍⬛

👀 Spontaneous chronic corneal epithelial defects (SCCEDs) can be frustrating to treat at the best of times, however French bulldogs are more likely to take longer to heal and suffer from further complications following diamond-burr debridement of their ulcers.

Why? It is proposed that genetics contribute to French bulldogs developing ulcers at a younger age, and that their brachycephalic conformation and physiology contributes to a significantly increased susceptibility to corneal infection. Additionally, the breed temperament can make post-operative medication application difficult which may also contribute to the development of complications and delayed healing.

Key points: be patient, ensure cases are adequately medicated, monitor patients closely for complications especially corneal infection, and be prepared to repeat debridement in some cases. Ask for advice or refer if possible if you think things are getting complicated!

💧Parotid duct transposition surgery is a good option for patients with dry eye where immunomodulatory therapy such as ciclosporin ointments or tacrolimus drops have not been effective, and where vision loss or ongoing ocular discomfort is a feature.

👉🏼 Lipogranulomatous Conjunctivitis in Cats

- Presents as smooth, nodular, white-yellow masses in the palpebral conjunctiva associated with the Meibomian glands
- Irritation caused by the masses may result in blepharospasm, ocular discharge and keratitis
- Mainly affects the upper eyelids but in some cases the lower eyelids are involved
- White cats are over-represented and it is thought solar-induced radiation damages the Meibomian glands
- Blockage of normal secretions from the Meibomian glands results in leakage of the secretions into the conjunctiva causing the inflammatory granulomas
- The lesions respond poorly to topical medications and surgical excision is the treatment of choice, with minimal recurrence noted
