🫵 THE DIAGNOSIS 🔎 | DIE AUFLÖSUNG

Did you identify the tendon? Habt ihr die Sehne erkannt?

This is a severe injury of the deep digital flexor tendon (DDFT) within the digital flexor tendon sheath.

Es handelt sich um eine schwerwiegende Verletzung der tiefen Beugesehne innerhalb der Fesselbeugesehnenscheide.

🔴 Red = affected deep digital flexor tendon
🔴 Rot = betroffene tiefe Beugesehne

A healthy tendon shows a dense, organized fibre architecture on ultrasound. Here, that normal architecture is severely disrupted (black areas) — indicating a major structural tendon injury.

Eine gesunde Sehne zeigt ultrasonografisch eine homogen organisierte Faserstruktur. Hier ist diese normale Architektur hochgradig gestört (dunkle Stellen = zerrissene Fasern) – ein Hinweis auf eine schwerwiegende strukturelle Sehnenverletzung.

WHAT DO THE RADIOGRAPHS TELL US? 🩻🔎

You’ve seen the radiographs. Now let’s take a closer look at the findings with

The right stifle shows marked periarticular soft tissue swelling and severe distension of both the femorotibial and femoropatellar joints. The infrapatellar fat pad is markedly effaced, and the patellar ligaments are poorly delineated.

But the key finding lies within the bone:

A large, well-defined osteolytic lesion is present within the subchondral bone of the lateral glenoid cavity of the proximal tibia, immediately adjacent to the articular surface. It is surrounded by a prominent sclerotic margin.

Interestingly, the distal femoral and proximal tibial physes show no radiographic evidence of involvement.

Sehnenverletzung 🐴 💉Nur acht Wochen nach der intraläsionalen Behandlung mit körpereigenem Platelet Rich Fibrin zeigt sich sonografisch bereits eine eindrückliche Auffüllung und Organisation der Sehnenläsion.

🐎Teil 4: zur Lokalisation des Schmerzes wurde eine Anästhesie der Nerven angebracht („abspritzen“).
Initial ist das Pferd vor der Anästhesie zu sehen und 10 Minuten nach der Leitungsanästhesie (MPA). Ist die Anästhesie positiv und somit 70% besser oder immer noch gleich lahm ? ABSTIMMEN 👇🙌🏽 weiteres zu diesem 🐎 folgt

🐎.Woher kommt die Lahmheit?

Leitungsanästhesien helfen uns, die schmerzhafte Region gezielt einzugrenzen: Bestimmte Abschnitte der Gliedmaße werden vorübergehend unempfindlich gemacht. Verbessert sich die Lahmheit um mindestens 70 %, suchen wir die Ursache im desensibilisierten Bereich weiter.
