Patient: 3–4 year old female cat.

Chief Complaint: No prior prenatal checks; owner knew she was pregnant and in labor, but she experienced prolonged dystocia (unable to deliver).

Diagnosis: Examination revealed two fetuses—one was still viable, while the other had no detectable heartbeat. Emergency C-section was advised and agreed upon.

Surgery: Emergency C-section was performed smoothly, taking about 10–15 minutes.

Fetal Status: Both kittens showed no signs of breathing or heartbeat (stillborn state) upon extraction.
Resuscitation: The medical team performed continuous CPR for over 20 minutes, successfully bringing one kitten back to life.

Served as a critical learning experience for new team members and interns, allowing them to witness a feline C-section and observe Day 0 newborn status firsthand.

We recently handled an urgent feline C-section case. A 3-to-4-year-old mom-to-be came in with dystocia. She had no prior prenatal exams, and despite showing clear signs of labor, she just couldn't deliver. A quick diagnostic scan showed two kittens—one was still holding on, but the other had already lost its heartbeat.

To save mom and the remaining baby, we immediately rushed into an emergency C-section.

The surgery itself was quick and smooth, taking only about 10 to 15 minutes. However, the moment we brought the two little ones out, our hearts sank—neither kitten had a pulse or breathing.

"Don't give up. Keep resuscitating!"

Our team kept going with continuous CPR. After more than 20 intense minutes, a faint, miraculous gasp finally came through. We managed to pull one of the kittens back from the brink. ❤️

For our clinic, every saved life is a win, but this case was extra special. It gave our new team members and interns their very first look at a live C-section and what a Day 0 newborn truly looks like in the ER.

Nature is tough, but a team that refuses to give up is tougher.

這天接到一個緊急的剖腹產病例。

一隻 3 到 4 歲的媽媽，因為先前沒有做過相關產檢，到了預產期雖然出現生產行為，卻一直無法順利產下寶寶。來院經過緊急檢查後發現，肚子裡的兩隻小貓，其中一隻已經失去了心跳。

為了保住媽媽與剩下的寶寶，我們迅速安排了緊急剖腹產。

雖然手術過程非常順利，大約 10 到 15 分鐘就完成，但當兩個小傢伙從子宮出來的那一刻，大家的心都沉了下來——兩隻寶寶當時都沒有呼吸與心跳。

「不要放棄，繼續復甦！」

醫療團隊沒有停下手中的動作，持續進行心肺復甦。經過超過 20 分鐘不間斷的努力，我們終於聽到了那聲微弱卻無比動人的呼吸聲，成功搶救回其中一隻小生命！

A Niu would like to introduce a case of GI foreign body in cats

GI foreign bodies in cats are true emergencies, especially because cats are uniquely prone to linear foreign bodies (string, thread, yarn) that can rapidly cause intestinal plication and perforation.

What Cats Commonly Swallow - String, yarn, thread, ribbon, tinsel, Hair ties, dental floss, Plastic pieces, toys, socks, jewelry

Cats are at particularly high risk for linear foreign bodies, which can anchor under the tongue or in the stomach and cause the intestines to “accordion” and tear.

🚨 Clinical signs include:
Persistent or intermittent vomiting
Anorexia or decreased appetite
Lethargy, hiding, restlessness
Drooling, gagging, or regurgitation
Abdominal pain or discomfort
Visible string from the mouth or a**s
Even early signs may be subtle, so any suspicion warrants same‑day evaluation.

🧪 Diagnosis
Physical exam, including mandatory check under the tongue for anchored string
Abdominal radiographs: segmental intestinal dilation; small intestine >1.2 cm raises suspicion
Exploratory surgery if obstruction is unclear or severe

🛠 Treatment
Endoscopic removal: ideal for stomach foreign bodies or,
Surgery (gastrotomy/enterotomy): required for most obstructions, especially linear FBs
Multiple enterotomies often needed for string cases
Bowel resection if perforation or necrosis

Supportive care: IV fluids, analgesia, anti‑nausea meds, antibiotics if peritonitis

📈 Prognosis
Excellent with early removal
Guarded if perforation, sepsis, or extensive resection
Hospitalization typically 1–3 days for simple cases; 5–7+ days for complicated ones

❌ What Owners Should Not Do
Do not pull visible string
Do not induce vomiting
Do not wait to “see if it passes”
These actions can worsen intestinal tearing.

✔️ Immediate Action
If a cat is vomiting, drooling, painful, or suspected of swallowing string or an object:
Seek veterinary care immediately. Early intervention saves lives.

Today, A Niu would like to share a case of pyometra + mammary gland tumor removal(clinical review)
If you have a non-spayed dog that starts drinking a lot, peeing a lot, is lethargic, and you see pus-like discharge coming out from the va**na, then you need to worry about pyometra.

Why Pyometra and Mammary Tumors Often Occur Together
Both diseases are strongly linked to chronic hormonal exposure, especially progesterone, in intact female dogs.

After each estrus, dogs enter diestrus, where progesterone remains elevated for 2–3 months.
Uterus: Progesterone → cystic endometrial hyperplasia (CEH) → bacterial infection → pyometra
Mammary glands: Progesterone → glandular proliferation → increased risk of mammary tumors

Result:
Older intact females have a high likelihood of developing both pyometra and mammary tumors.

Clinical Relationship Between the Two Diseases
High coexistence rate
Studies show:

Over 50% of dogs with mammary tumors also have CEH or uterine abnormalities.
Pyometra is more common in older intact females — the same population at highest risk for mammary tumors.

Estrus cycle influence
Progesterone stimulates mammary tissue → tumor growth
Progesterone also drives uterine changes → CEH → pyometra

Systemic signs: lethargy, PU/PD, vomiting, va**nal discharge → suspect pyometra

Diagnostic Approach When Both Conditions Are Suspected
Essential diagnostics
CBC: leukocytosis, toxic changes
Biochemistry: renal and hepatic parameters
Abdominal ultrasound: enlarged, fluid-filled uterus
Thoracic radiographs: check for metastasis

Key point:
If a dog with mammary tumors shows systemic illness, always rule out pyometra.

Treatment Strategy
⭐ Priority: Treat Pyometra First (Life-threatening)
Emergency ovariohysterectomy (OHE)
Removes infection source and stabilizes the patient
Some mammary masses near the incision may be removed simultaneously

Mammary tumor surgery (second stage)
Performed once the patient is stable
Allows proper staging and surgical planning

Why not treat the mammary tumor first?
Pyometra can rapidly lead to sepsis, renal failure, uterine rupture, death
Mammary tumors are usually chronic, not immediately life-threatening
Anesthetic risk is significantly higher during active pyometra

Mammary tumors
About 50% are malignant
Early removal + OHE reduces recurrence and new tumor formation

OHE is both therapeutic and preventive for both conditions.

臨床上常見情境
老年未結紮母犬+乳腺腫瘤多年未處理

🩻 3. 診斷策略（兩者同時存在時）
必做檢查：
CBC：白血球上升、毒素性變化
Biochem：腎指數、肝指數
Abdominal ultrasound：子宮擴張、液體
Thoracic radiographs：乳腺腫瘤是否轉移
🏥 4. 治療策略（臨床決策重點）
⭐ 若同時有 Pyometra + Mammary tumor → 優先處理 Pyometra（救命）

❗ 為什麼不先做乳腺腫瘤？
Pyometra 是 急症，可能敗血症、腎衰竭、子宮破裂
乳腺腫瘤通常是 慢性病，不會立即致命
手術風險在 Pyometra 時更高（麻醉、循環不穩）

📈 5. 預後
Pyometra：
早期手術 → 預後良好
早期切除 + OHE 可降低復發與新腫瘤形成
若已轉移 → 預後差

OHE 是兩者最有效的預防與治療輔助方式

ANVH EP- 98 Third Eyelid Gland Prolapse in Cats 貓的第三眼瞼外翻

The third eyelid (nictitating membrane) contains a tear gland responsible for producing about 30–40% of total tear volume.
When the connective tissue anchoring this gland becomes loose or inflamed, the gland protrudes from the inner corner of the eye as a red, rounded mass.

Cherry eye is less common in cats than in dogs, but it does occur—especially in:
Persian and Exotic Shorthair cats
Cats with chronic conjunctivitis
Cats with eyelid or third eyelid laxity
Rarely, cats with third eyelid tumors or foreign body irritation

📌 Clinical signs
Red, round swelling at the inner corner of the eye
Increased tearing or discharge
Conjunctivitis
Ocular irritation or pawing at the eye
Sometimes accompanied by third eyelid elevation or eyelid inflammation

📌 Why the gland should NOT be removed
The third eyelid gland produces a significant portion of tears.
Removing it can lead to keratoconjunctivitis sicca (KCS), which is harder to manage in cats and may cause:
Chronic corneal inflammation
Corneal ulceration
Vision impairment
Lifelong medication

Therefore, preservation and surgical repositioning of the gland is the standard of care.

📌 Treatment
1. Surgical replacement (gold standard)
Techniques include:
Pocket technique
Anchoring technique
Surgery in cats is more delicate than in dogs due to thinner tissues and a smaller third eyelid, but success rates are high.

2. Medical support
Topical antibiotics
Anti‑inflammatory eye medications (as appropriate)
Elizabethan collar to prevent rubbing
Medication alone cannot correct the prolapse.

切除會導致 乾眼症（KCS），在貓的治療難度更高，且可能造成：
終身點藥/慢性角膜炎/角膜潰瘍跟視力下降

📌 治療原則
1. 手術復位（標準）
Pocket technique（囊袋法）
Anchoring technique（固定法）
貓的組織較薄、瞬膜較小，手術比狗更精細，但成功率仍高。

2. 藥物輔助
抗生素眼藥
抗發炎眼藥（依醫師判斷）
避免揉眼（伊莉莎白圈）

📌 特別注意：貓的鑑別診斷
貓的第三眼瞼腫塊 不一定是腺體外翻，還可能是：

Today, A Niu would like to introduce a very rare case - intussusception in cats.

"Little Black" had GI symptoms for over 3 weeks; the cat was extremely dehydrated and lethargic when we first saw it. After several examinations, our diagnosis is intussusception. Discuss with O, then decide to perform exploratory surgery to fix it.

Intussusception in cats is an acute, potentially life‑threatening gastrointestinal obstruction caused by one segment of intestine telescoping into another. It most often affects the jejunum or ileocecocolic junction and is usually secondary to underlying intestinal disease.

Causes: GI inflammation, parasites, infections, foreign bodies, IBD, motility disorders.

Clinical Signs: Vomiting, diarrhea or bloody stool, abdominal pain, lethargy, palpable tubular mass.

Diagnosis: Ultrasound showing “target/bull’s‑eye” sign; radiographs; f***l tests.

Treatment: Surgical reduction or resection, and correct underlying disease.

Prognosis: Good with early surgery; worse if necrotic or perforated.

手術復位或腸切除，並處理造成腸套疊的原發疾病。

若及早手術，預後良好；若已壞死或穿孔，風險大幅增加。

這個case最困難的地方在於腹腔打開來的時候常到已經壞死且破裂,腸道內容物已經散到腹腔裡;判斷不能整復後即施行截腸跟腸吻合手術

這對貓咪來說是個重大的手術,截掉約20-25%的腸管,吻合後要面臨的是腹膜炎的挑戰;埋了食道胃管,住院住了5天,硬是把這貓從鬼門關裡拉了回來

通常腸套疊發生在狗,而且是年紀較輕的狗,在中年貓咪是非常罕見的,加上手術難度高,再來主人沒有放棄,願意埋食道管+住院,才給這隻貓咪第2次重生的機會

Today, A Niu would like to share a case of a spleen tumor in dogs

Splenic tumors in dogs are more common than many owners realize. They can stay hidden until they suddenly bleed, causing weakness, pale gums, or even collapse.

About ⅓ are benign, but ⅔ are malignant, so any splenic mass is an Emergency.

Treatment is usually splenectomy, and dogs recover well from surgery. If your dog shows sudden weakness or abdominal swelling, seek veterinary care right away.

ANVH EP- 95 Esophagus foreign body in dogs 狗的食道異物

A small dog ate a huge treat. Since then, the dog has started to drool and become restless.

🐶 Esophageal Foreign Body in Dogs — Act Fast!
Dogs may swallow bones, rawhide, toys, or fishhooks that get stuck in the esophagus. This is an emergency.

Common signs:
• Regurgitation
• Gagging/retching
• Drooling
• Pain when swallowing
• Repeated attempts to swallow

What to do:
Seek immediate veterinary care. X‑rays and endoscopy are usually needed to confirm the FB location, then remove the object.

Why it’s urgent:
Delays can cause esophagitis, perforation, infection, or strictures.

Prognosis:
Excellent if removed early.

This is a classic urgent condition, if your dog has simliar issue, please contact your family vet ASAP.

ANVH EP- 94 Hypertrophic CardioMyopathy (HCM) 貓的肥厚性心肌病

Hypertrophic cardiomyopathy (HCM) is the most common cardiac disease in cats, characterized by pathologic thickening of the left ventricular myocardium, leading to diastolic dysfunction, left atrial enlargement, congestive heart failure, and thromboembolism.

A Niu would like to share a case of a sudden faint during grooming, and started to have difficulty breathing.

🫀 What HCM is
HCM = primary myocardial disease with concentric hypertrophy of the LV without identifiable systemic cause.

🐈 Clinical signs
Many cats are asymptomatic until decompensation.

Early/subclinical
Incidental murmur, gallop, or tachycardia
No outward signs

Decompensated disease includes:
CHF: tachypnea, dyspnea, pleural effusion, pulmonary edema
ATE: acute hindlimb paresis/paralysis, pain, cold limbs
Syncope or sudden death

🔍 Diagnosis
1. Echocardiography (gold standard)
2. Rule out secondary causes
Hyperthyroidism
Systemic hypertension
Acromegaly (in older male cats with diabetes)
3. Biomarkers
NT-proBNP (SNAP or quantitative)
Useful for screening or differentiating cardiac vs respiratory distress
4. Thoracic radiographs
Evaluate for CHF, not reliable for diagnosing HCM itself
5. ECG
May show arrhythmias (VPCs, atrial arrhythmias)

💊 Treatment (based on clinical stage)
Need long-term medication to control it

📈 Prognosis
Highly variable:
Asymptomatic: years
CHF: median survival ~6–18 months
ATE: median survival ~2–6 months (if they survive initial event)
Sudden death is possible at any stage

Negative prognostic indicators:
Large LA/CHF at diagnosis/ATE/Hypothermia/High NT-proBNP

🫀 什麼是 HCM
HCM 是一種原發性心肌疾病，其特點為左心室向心性肥厚，且無明確的系統性原因可解釋（如高血壓或甲狀腺機能亢進）。

🐈 臨床症狀
許多貓在疾病早期完全沒有症狀，直到突然惡化。
早期 / 亞臨床階段偶然聽到的 心雜音、奔馬律 或 心搏過速
外觀上通常無明顯異常
但當疾病惡化時會出現: 呼吸急促/呼吸困難/胸腔積液/肺水腫/動脈血栓栓塞（ATE）/後肢急性癱瘓或無力/四肢冰冷、疼痛/昏厥（syncope）或猝死

在美容時突然昏倒、隨後呼吸困難，常見於急性 CHF 或心律不整 所引起的 HCM 惡化。

🔍 診斷方式
1. 心臟超音波（黃金標準）
2. 排除次發性原因
甲狀腺機能亢進及全身性高血壓
肢端肥大症（Acromegaly）：常見於年長糖尿病公貓
3. 心臟生物標記
NT‑proBNP（快篩或定量）
4. 胸腔 X 光
5. 心電圖（ECG）

💊 治療（依疾病階段）
通常需要長期藥物控制預防症狀發作
預後保守，復發率高
大多數有結構性心臟病變的貓需要長期甚至終身用藥與定期追蹤。

📈 預後
依疾病階段差異極大：
無症狀：可存活 數年
CHF：中位存活 6–18 個月
ATE：中位存活 2–6 個月（若度過急性期）
任何階段皆可能猝死

不良預後指標為左心房明顯擴大/初診即為 CHF/ATE/體溫偏低/NT‑proBNP 升高
