Pour identifier précisément l’origine du problème, nous réalisons un test intradermique (IDT), considéré comme la référence en matière de diagnostic des allergies chez le cheval. Lors de cet examen, de petites quantités de jusqu’à 60 allergènes différents sont injectées juste sous la peau. Parmi eux figurent les pollens de graminées et d’arbres, les acariens de poussière et de stockage, les moisissures ainsi que certains insectes tels que les Culicoides (responsables de la dermite estivale récidivante).

En évaluant les réactions cutanées à différents intervalles de temps, nous pouvons déterminer avec précision quelles substances déclenchent la réaction allergique.

Un diagnostic précis nous permet ensuite de proposer un traitement ciblé : adaptation de l’environnement du cheval, mise en place d’une immunothérapie spécifique aux allergènes (vaccin de désensibilisation personnalisé), ou combinaison des deux. Dans la majorité des cas, une amélioration significative et durable est possible.

Om de oorzaak van het probleem te achterhalen, maken we gebruik van een intradermale huidtest (IDT), de gouden standaard binnen de allergiediagnostiek bij paarden. Tijdens deze procedure worden kleine hoeveelheden van maximaal 60 verschillende allergenen net onder de huid geïnjecteerd. Les mélanomes équins sont entourés de nombreuses idées reçues. L'une des plus répandues est la croyance selon laquelle ces tumeurs ne peuvent pas être traitées, ou que leur ablation chirurgicale accélérerait les récidives. Cela est sans fondement.

À Equitom, nous proposons des solutions chirurgicales pour les mélanomes à tous les stades, y compris pour les lésions petites et précoces. Intervenir tôt permet précisément de réaliser un geste moins invasif, d'obtenir de meilleurs résultats et de limiter le risque de complications locales. Attendre qu'une tumeur devienne volumineuse ou symptomatique ne fait que compliquer la prise en charge et peut conduire à des interventions plus lourdes, comme une amputation de la queue.

Equine melanomas are surrounded by persistent misconceptions. One of the most common is the belief that these tumors cannot be treated, or that surgical removal would accelerate recurrence. This is unfounded.

At Equitom, we offer surgical solutions for melanomas at every stage, including early, small lesions. Intervening early is precisely what allows for a less invasive procedure, better outcomes, and a reduced risk of local complications. Waiting until a tumor becomes large or symptomatic only complicates management and may ultimately lead to more extensive interventions such as tail amputation.

More than 80% of grey horses over the age of 15 develop melanomas. While many remain benign for years, these tumors can progress, cause discomfort, or become malignant if left untreated. Early detection and timely intervention are therefore essential, regardless of lesion size.

High-risk areas to monitor regularly include the tail base, perineal region, lips, and parotid area. Any change in size, shape, or appearance warrants a veterinary consultation without delay.

Net vroegtijdig ingrijpen maakt een minder invasieve ingreep mogelijk, leidt tot betere resultaten en beperkt het risico op lokale complicaties. 🚨Une masse sur l’oeil 🐴

Ce cheval haflinger a été référé à Equitom suite à l’apparition d’un carcinome sur son oeil droit.

➡️ Les carcinomes à cellules squameuses oculaires sont plus fréquents chez les haflingers via une mutation génétique. Il s’agit d’un cancer localisé envahissant, accentué par l’exposition au soleil.

😷Dr. Sauvage et notre équipe ont procédé à une kératectomie lamellaire et une conjonctivoplastie afin de retirer la tumeur, son oeil droit étant déjà sévèrement atteint.
Un raccourcissement des paupières a également été réalisé en prévention aux deux yeux pour limiter l’exposition des conjonctives au soleil.

Même si la cicatrisation se passe bien, le risque de récidives ou de nouveaux carcinomes est malheureusement bien réel. C’est pourquoi le port d’un masque anti-UV est fortement recommandé dans ce cas-ci de manière préventive.

Nous souhaitons un bon rétablissement à ce courageux cheval.💙

This Haflinger horse was referred to Equitom following the appearance of a carcinoma on his right eye.

➡️ Ocular squamous cell carcinomas are more common in Haflingers due to a genetic mutation. It is a locally invasive cancer, worsened by sun exposure.

😷 Dr. Sauvage and our team performed a lamellar keratectomy and conjunctivoplasty to remove the tumour, as his right eye was already severely affected.
A eyelid shortening procedure was also carried out preventively on both eyes to limit conjunctival exposure to the sun.

Although healing is progressing well, the risk of recurrence or new carcinomas is unfortunately very real. That is why wearing a UV-protective mask is strongly recommended in this case as a preventive measure. We wish this brave horse a smooth recovery.

➡️ Oculaire plaveiselcelcarcinomen komen vaker voor bij Haflingers door een genetische mutatie. Het betreft een lokaal invasieve kanker, versterkt door blootstelling aan de zon.

😷 Dr. Sauvage en ons team voerden een lamellaire keratectomie en conjunctivoplastiek uit om de tumor te verwijderen, aangezien zijn rechteroog al ernstig was aangetast.
Een verkorting van de oogleden werd eveneens preventief uitgevoerd aan beide ogen om de blootstelling van de conjunctiva aan de zon te beperken.

Ook al verloopt de genezing goed, het risico op herval of nieuwe carcinomen is helaas zeer reëel. Daarom wordt het dragen van een UV-beschermend masker in dit geval sterk aanbevolen als preventieve maatregel. We wensen dit dappere paard een voorspoedig herstel. 💙

Deux testicules cachés. Une chirurgie mini-invasive.
Cet étalon a été référé chez Equitom pour un cryptorchidisme abdominal bilatéral : une affection dans laquelle les deux testicules ne descendent pas dans le sc***um et restent cachés dans la cavité abdominale.
Bien qu’invisibles de l’extérieur, des testicules retenus peuvent continuer à produire des hormones et sont souvent associés à un comportement persistant d’étalon, des problèmes de fertilité ainsi qu’à des risques pour la santé à long terme.
🩺 Comme aucun des deux testicules n’était accessible via le canal inguinal, notre équipe chirurgicale a réalisé une cryptorchidectomie laparoscopique sur cheval debout, sous anesthésie locale.
Grâce à cette approche mini-invasive, de petites ouvertures ont permis d’introduire une caméra et les instruments chirurgicaux dans l’abdomen. Les deux testicules retenus ont ensuite été localisés avec précision, ligaturés puis retirés avec soin. Cette technique offre une excellente visualisation de la cavité abdominale tout en limitant les traumatismes tissulaires et le gonflement postopératoire.
🙏 L’intervention s’est déroulée sans complication et les deux testicules retenus dans l’abdomen ont pu être retirés avec succès.

Two hidden testicles. One minimally invasive surgery.
This stallion was referred to Equitom for a bilateral abdominal cryptorchidism: a condition where both testicles fail to descend into the sc***um and remain hidden inside the abdomen.
Although invisible from the outside, retained testicles can continue producing hormones and are often associated with persistent stallion-like behaviour, fertility issues and long-term health risks.
🩺 Because neither testicle was accessible through the inguinal canal, our surgical team performed a standing laparoscopic cryptorchidectomy under local anaesthesia.
Using a minimally invasive approach, small portals were created to introduce the camera and surgical instruments into the abdomen. Both retained testicles were carefully identified and removed with precision. This minimally invasive technique provides excellent visualization inside the abdomen while minimizing tissue trauma and postoperative swelling.
🙏The procedure went smoothly, and both abdominally retained testicles were successfully removed without complications.

Deze hengst werd doorverwezen naar Equitom met bilaterale abdominale cryptorchidie: een aandoening waarbij beide testikels niet indalen in het sc***um en verborgen blijven in de buikholte.
Hoewel ze van buitenaf niet zichtbaar zijn, kunnen achtergebleven testikels hormonen blijven produceren. Dit gaat vaak gepaard met aanhoudend hengstengedrag, vruchtbaarheidsproblemen en gezondheidsrisico’s op lange termijn.
🩺 Omdat geen van beide testikels bereikbaar was via het lieskanaal, voerde ons chirurgisch team een laparoscopische cryptorchidectomie uit bij het staande paard onder lokale verdoving.
Met behulp van een minimaal invasieve techniek werden kleine toegangspoorten gemaakt om de camera en chirurgische instrumenten in de buikholte te brengen. Beide achtergebleven testikels werden zorgvuldig opgespoord en met grote precisie verwijderd. Deze techniek biedt een uitstekend zicht in de buikholte en beperkt tegelijkertijd weefselschade en postoperatieve zwelling tot een minimum.
🙏 De ingreep verliep vlot en beide intra-abdominaal gelegen testikels werden succesvol verwijderd zonder complicaties.

🇨🇳🐴 Une nouvelle intervention chirurgicale enrichissante en Chine - suite de la vidéo diagnostic

Lors de son récent séjour en Chine, Tom Mariën a eu l’opportunité d’opérer un cheval de sport de haut niveau souffrant d’une boiterie chronique.

Grâce à une ténoscopie, il a pu examiner l’intérieur de la gaine tendineuse et identifier l’origine du problème : une déchirure longitudinale située sur le bord externe du tendon fléchisseur profond du doigt. Les fibres tendineuses endommagées ont été soigneusement retirées à l’aide d’un résecteur synovial, puis la surface du tendon a été traitée par radiofréquence afin de créer un environnement de cicatrisation plus lisse et plus stable.

L’intervention s’est déroulée avec succès et le cheval s’est réveillé sans complication. Il poursuit désormais son programme de rééducation.

Ce qui rend ce type de cas particulièrement gratifiant, ce n’est pas seulement d’aider un cheval à retrouver son confort et ses performances, mais aussi de pouvoir partager son expertise et collaborer avec des confrères talentueux à travers le monde. La médecine équine ne connaît pas de frontières, et c’est toujours un privilège de contribuer à l’évolution de la profession au-delà de sa propre clinique et de son propre pays.

Another rewarding surgical case in China 🇨🇳🐴
Follow up of the diagnostic in our last video

During his recent visit to China, Tom Mariën had the opportunity to operate on a top-level sport horse suffering from chronic lameness.

Using a tenoscopic procedure, he examined the inside of the tendon sheath and identified the source of the problem: a longitudinal tear along the outer border of the deep digital flexor tendon. The damaged tendon fibres were carefully removed using a synovial resector, after which the tendon surface was treated with a radiofrequency probe to create a smoother and more stable healing environment.

The surgery was successful, and the horse recovered smoothly from anesthesia. It is now progressing through its rehabilitation programme.

What makes cases like these particularly rewarding is not only helping a horse on its path back to soundness and performance, but also the opportunity to share expertise and collaborate with talented colleagues around the world. Equine medicine knows no borders, and it is always a privilege for him to contribute to the advancement of the profession beyond his own clinic and country.

We wish this horse a smooth recovery and a successful return to competition.

Nog een mooie chirurgische case in China 🇨🇳🐴

Tijdens zijn recente bezoek aan China kreeg Tom Mariën de kans om een topsportpaard te opereren dat al geruime tijd kampte met chronische kreupelheid.

Met behulp van een tenoscopische procedure onderzocht hij de binnenzijde van de peesschede en kon hij de oorzaak van het probleem vaststellen: een longitudinale scheur langs de buitenzijde van de diepe buigpees. Dit onderzoek is al gepubliceerd op onze kanalen. De beschadigde peesvezels werden zorgvuldig verwijderd met behulp van een synoviale resector. Vervolgens werd het peesoppervlak behandeld met een radiofrequente probe om een gladder en stabieler genezingsoppervlak te creëren.

De operatie verliep succesvol en het paard herstelde vlot van de anesthesie. Momenteel doorloopt het zijn revalidatieprogramma.

Wat dit soort gevallen bijzonder waardevol maakt, is niet alleen de mogelijkheid om een paard te helpen op weg naar herstel en een terugkeer naar sportprestaties, maar ook de kans om kennis te delen en samen te werken met getalenteerde collega’s wereldwijd. Paardengeneeskunde kent geen grenzen en het is telkens opnieuw een voorrecht om bij te dragen aan de verdere ontwikkeling van het vakgebied, ook buiten de eigen kliniek en het eigen land.

Une chirurgie tendineuse au-delà des frontières

Lorsqu’un cheval a besoin de soins médicaux avancés et que son propriétaire se trouve à l’autre bout du monde, l’incertitude peut être particulièrement difficile à vivre. Dans ces situations, une communication claire devient essentielle, surtout lorsque les examens révèlent un problème qui ne peut pas se résoudre seul et nécessite une intervention chirurgicale.

Lors d’une récente visite dans une clinique équine en Chine, le Dr Tom Mariën, fondateur d’Equitom et d’Equine Care Group, a examiné un cheval dont le propriétaire ne pouvait pas être présent. Le cheval présentait une inflammation chronique du tendon, accompagnée d’adhérences et de tissu prolifératif. Ce type de lésions ne disparaît pas spontanément et ne répond pas suffisamment aux infiltrations seules.

Après un examen orthopédique complet, une échographie et un bilan diagnostique approfondi, le propriétaire a été contacté afin de lui expliquer le diagnostic ainsi que les différentes options thérapeutiques possibles.

En concertation avec le propriétaire, nous avons décidé de procéder à une chirurgie arthroscopique.

Nous sommes heureux d’annoncer que l’intervention s’est bien déroulée et nous remercions sincèrement le propriétaire pour la confiance accordée à notre équipe.

Tendon Surgery Across Borders

When a horse needs advanced medical care and the owner is far away, uncertainty can be incredibly difficult. Clear communication becomes essential: especially when diagnostics reveal a problem that won’t resolve on its own and need surgery.

During a recent visit to a Chinese equine clinic, Dr. Tom Mariën, founder of Equitom and Equine Care Group, examined a horse whose owner could not be present. In this case, the horse was diagnosed with chronic tendon inflammation, accompanied by adhesions and proliferative tissue. These changes do not disappear spontaneously, nor do they respond sufficiently to infiltrations.

After a full orthopedic examination, ultrasound and diagnostic work‑up, the owner of the horse was called and informed of the diagnosis and possible next steps.

In concertation with the owner we proceeded with arthroscopic surgery.

Peeschirurgie over de grenzen heen

Wanneer een paard geavanceerde medische zorg nodig heeft en de eigenaar zich ver weg bevindt, kan de onzekerheid bijzonder moeilijk zijn. Duidelijke communicatie wordt dan essentieel, zeker wanneer onderzoek een probleem aan het licht brengt dat niet vanzelf verdwijnt en een operatie nodig heeft.

Tijdens een recent bezoek aan een Chinese paardenkliniek onderzocht Dr. Tom Mariën, oprichter van Equitom en Equine Care Group, een paard waarvan de eigenaar niet aanwezig kon zijn. In dit geval werd bij het paard een chronische peesontsteking vastgesteld, gepaard met verklevingen en proliferatief weefsel. Deze veranderingen verdwijnen niet spontaan en reageren onvoldoende op infiltraties.

Na een volledig orthopedisch onderzoek, echografie en verdere diagnostische evaluatie werd de eigenaar van het paard telefonisch geïnformeerd over de diagnose en de mogelijke vervolgstappen.

In overleg met de eigenaar gingen we over tot een arthroscopische ingreep.

We zijn blij te kunnen melden dat de operatie succesvol verliep en zijn dankbaar voor het vertrouwen dat de eigenaar in ons stelde.

La thérapie ACP chez le cheval

La médecine régénérative joue un rôle de plus en plus important dans la médecine sportive équine moderne et la rééducation. L’un des traitements les plus utilisés aujourd’hui est la thérapie ACP : un traitement biologique basé sur le propre sang du cheval.

L’ACP (Autologous Conditioned Plasma) est un produit sanguin concentré riche en plaquettes et en facteurs de croissance. Ces facteurs de croissance jouent un rôle important dans la réparation des tissus, la cicatrisation et la réduction de l’inflammation.
