Furkids Funhouse

I give permission for my dog to be group-housed with other dogs during daycare, noting that staff exercise great care to ensure that “playmates” are compatible. I accept that there are inherent risks associated with a daycare environment, which could include such things as: transfer of communicable illness such as canine cough and Injuries such as bites, cuts and abrasions and broken or split nai

2.I declare that my dog is in good health and has shown no signs of illness or injury in the past 14 days. I will contact Furkids Funhouse immediately if this changes.
3. I declare that my dog is suitable to be with other dogs and has not shown aggressive or threatening behaviour towards any person, animal or thing. Occasionally a dogs behaviour can change and I accept that Furkids Funhouse have the right to temporarily or permanently remove my dog from daycare at any time should they find that daycare is no longer an appropriate environment for my dog.
4. I agree that I am solely responsible for any loss, damage or harm caused or contributed by my dog while he/she is attending Furkids Funhouse
5. Whilst my dog is in the care of Furkids Funhouse, and in the case that I or my emergency contact cannot be contacted and/or in the event of an emergency, Furkids Funhouse and its staff may seek immediate veterinary care for my dog. I acknowledge that I am responsible for all costs associated with such vetinary care and will provide Furkids Funhouse with my payment details (such as debit or credit card) so that it may be used for the payment of such costs
6. In the case of an emergency, I give permission for my pet to be transported in a vehicle to seek veterinary or other professional attention as above or for evacuation in an emergency.
7. I give permission for medication to be given to my pet by staff according to the schedule you provide, or according to veterinary direction.
8. I release Furkids Funhouse, and its employees, from all liability, and indemnify Furkids Funhouse from any and all claims of every kind and nature whatsoever, arising directly and indirectly, from my dogs participation in activities or use of service, including any legal claims and medical expenses.
9. I accept that photos of my dog playing at Furkids Funhouse will be posted on social media, including but not limited to Facebook and Instagram and/or used on website or marketing.
10. I accept that my dog must be picked up by closing time, being 6pm. A late fee of $15.00 for the first 10 minutes after close time (being 6pm) and then a further $20 for each 10-minute block until your dog is picked up. Fee must be paid at time of collection of your dog. If I do not collect my dog as arranged and I do not make further arrangement with Furkids Funhouse for the ongoing care of my pet/s WITHIN 2 DAYS of this date, that Furkids Funhouse will consider that I have abandoned my dog and will seek foster care/shelter or council care of the dog.
11. Cancelation fees: I agree to pay all cancellation fees where I do not provide 24 hours’ notice to cancel a service that was booked or an ongoing booking. The fee is $20 per service (daycare and/or grooming). Maximum cancellation fee for a daycare plus grooming booking that is cancelled with less than 24 hours’ notice is $40.00. By submitting an admission form and enrolling your dog into Furkids Funhouse or booking a grooming service, you are agreeing that you have read and agree to this agreement.
