🦷 Can adding buprenorphine to an infraorbital nerve block extend analgesia beyond 48 hours in canine dental patients?

A recent randomised crossover study in dogs compared a standard bupivacaine block versus a buprenorphine-augmented block over 5 days of MAC monitoring. The headline result was P=0.775 — technically "not significant." But the data tells a more nuanced story.

The key findings:
📊 At 48h: 50% of buprenorphine dogs still had reduced MAC vs 25% of bupivacaine-only dogs
⏱ At 96h: bupivacaine effect was ZERO in all subjects — 25% of buprenorphine dogs were STILL covered
💡 Bupivacaine alone lasted 24–72h — far longer than the textbook 4–8h

Why did it "fail"?
→ N=8 (needed ~40 per group for power)
→ Healthy subjects = minimal peripheral mu receptor upregulation
→ Supramaximal dolorimetry may mask subtle analgesic effects

Bottom line: statistical insignificance ≠ clinical irrelevance. The adjunct is safest candidates are patients with chronic periodontal or oral inflammatory disease — where peripheral mu receptors are upregulated and most likely to respond.

Haemodynamics? Statistically significant HR and MAP trends — but all within acceptable clinical limits. Safe protocol.

🐾 Choosing the Right Anaesthesia Equipment for Your Patient – Key Principles!
Patient safety during anaesthesia starts with choosing the right equipment. Body weight is everything – here's a quick reminder:
Breathing Circuits:

< 3 kg → Neonatal Mapleson D
3–10 kg → Small circular circuit
> 10 kg → Large circular circuit

Always remember:
✅ Match the rebreathing bag to your patient (3–5 tidal volumes)
✅ Always perform a leak test before every procedure
✅ Set fresh gas flow (FGF) correctly for the circuit type
✅ Keep dead space in check – max. 20% of the patient's tidal volume
A well-prepared anaesthesia station means peace of mind – for the patient and for us. 🏥🐕🐈
